All vendors must provide a vendor registration packet to VUSD in order to provide goods or services. Vendor registration packets must be completed electronically and a current IRS Form W-9 must be provided to VUSD each year. 

Vendor Registration Packet with Contact Information 

• A purchase order from Ventura Unified Purchasing department must be received by a vendor in order to begin work, ship goods, or provide services to the district. Verbal contracts, agreements or other promises are not valid agreements and will not be accepted.
• Any changes require a formal change order to the original purchase order and no work/services/goods should be provided until the change order is received by the vendor. Ventura Unified is subject to sales tax on tangible goods.
• The current sales tax rate for the City of Ventura should be included on quotes and invoices (2023 rate - 7.75% sales tax).
• Vendors must provide packing slips in all shipments that include the corresponding purchase order listed clearly on the packing slip. Shipments without purchase order numbers may be refused.
• Shipments must be made to address on purchase order. Goods shipped to alternative locations will be refused. 

All vendors must complete and execute a service agreement with VUSD before any work or services may commence. 

• VUSD Service Contract - Complete (Short) Form     - For all vendors who are providing services which are not on a VUSD campus or site.

California Student Data Privacy Agreement (CSDPA). 

All vendors who are providing a service or software that involves student access or data must have an executed VUSD CSDPA before any services or work may commence without exception. Ventura USD has contracted with Colbi Technologies to provide a web-based process for submitting to their Prequalified Contractor and CUPCCAA lists called Quality Bidders
