Enrollment Packet and Next Steps

 

Dear Parent(s) and Guardian(s):

 

Welcome to Jumpstart!

 

This is Cara Dallamora, Early Childhood Education Coordinator. Thank you for your interest in our Program. Completion and verification of required forms and documents is required in order to enroll your child and to ensure that they keep their placement.

 

Please follow the steps below.  It is important to complete these steps as quickly as possible so that we may try and accommodate all interested families on our wait list.

 

I.   Enrollment Packet

  • Pick up an enrollment packet from the District Office between 8 a.m. – 4 p.m. (see address below)
    • When you arrive, please ask the front desk for Nikki or Cara at Jumpstart

  • Print and complete the forms from this page

II.   Enrollment Meeting

 

  • Once you have completed the enrollment packet, go to the following link to choose a date and time to schedule an enrollment meeting: https://calendly.com/cara-dallamora-venturausd/30min
  • At the meeting, in addition to the forms in the enrollment packet, you will need the required documents listed below which will remain confidential:
    • Recent total  income of the last month (two incomes if applicable) – (for example, check stubs, cash aid, CalWORKs, etc.)
    • *Proof of vaccinations for your child (a form for a physical is included with the enrollment packet)
    • Original or certified copy of birth certificate or hospital certificate of birth for your child
    • 2 proofs of address (for example, utility bill, bank statement, rental or mortgage statement, etc.)
    • Picture identification for parent/s
    • Proof of other children in household for family size (for example, birth certificate, hospital record, passport, etc.)

 

Please note: If you have not completed all of your paperwork you may continue with the enrollment meeting and we will support you with completion. *The physical form must be completed by the first day of school or your child will not be able to start.
 

III.   Pre-enrollment Form
 

  • New and returning students, if you have not done so already:
  • Go to the following link to pre-enroll: https://venturausd.vcoe.org/preenrollment/
  • Choose New Online Registration or Continue Online Registration
  • Hit the submit button after each section
  • Hit the submit button on the top of the page once all sections have been completed
  • Print out and sign the form to bring to your enrollment meeting
