ORIGINAL BIRTH CERTIFICATE & ONE COPY
(MUST BE 5 YEARS OLD ON OR BEFORE OCTOBER 1ST)
ORIGINAL SHOT RECORD & ONE COPY
(polio, hepatitis B, diphtheria, tetanus, pertussis, measles, mumps and rubella, and varicella {chickenpox}
Upon first enrollment, students entering Kindergarten are required to have a school entry health examination.
This exam should be give no earlier than March 1 prior to the start of Kindergarten.
