Kindergarten Registration

Kindergarten Registration begins Wednesday, March 24, 2010 from 1:00-3:00 PM.

We will continue to register students each day after the 24th from 9:30 AM - 1:00 PM.

Please call us at (805) 289-1734 if you need to schedule another time for registration.

PLEASE PROVIDE THE FOLLOWING:

Proof of residency:  The original and one copy of current utility bills (i.e. water, electric, gas) or Driver License with correct address and (1) current utility bill, or if you are moving into the area you MUST have your escrow papers which should have your actual address on them along with your signature.

Original Legal Birth Certificate and one copy (not hospital certificate), Baptismal record or Passport.  

Kindergarten - Must be born on or before Dec. 2, 2005

Report of Health Examination for School Entry (Grades Kdg. and 1st only) - this form must be completed by a physician and

submitted to the Elementary School office prior to the start of school.

Official Immunization record and one copy signed by a doctor's office or clinic.

Dental Exam due by May, 2011 (Kindergarten only)
